About The Position

Fairview Masonic Children's University Hospital is hiring a 0.8 (64 hours per pay period) RN Care Coordinator Working Supervisor to oversee our Pediatric Care Coordinator team! The Supervisor, Care Coordination is responsible for supervising the day to day operations of Care Coordinators. Provides workflow direction and ensures standard work performance that is consistent with achieving care coordination organizational objectives. Other duties include achieving standardization and integration of care coordination team and the daily engagement system. They will assist in the development and implementation of policies and procedure, strategic planning, optimal use of resources and leading and/or participating in process improvement projects. Provides leadership strategy (~30%) and direct patient care coordination (~70%) based department need as identified and managed by the role.

Requirements

  • Bachelor of Science in Nursing (BSN)
  • 5 years Registered Nurse (RN) clinical experience in acute care hospital setting
  • 3 years working as a care coordinator/case manager
  • Have an understanding of hospital, community resources and resource/utilization management.
  • Have working knowledge of use of evidence based guidelines.
  • Demonstrate critical thinking skills, problem-solving abilities, effective communication skills and time management skills.
  • Demonstrate ability to work effectively on an interdisciplinary team.
  • Have familiarity with computer systems and Microsoft applications.
  • Be available/able to work flexible hours, including covering weekends, and work on call as assigned.

Nice To Haves

  • 5 years RN clinical experience in acute care hospital setting
  • 5 years care coordination/case manager
  • Basic Life Support (American Heart Assoc or Red Cross)
  • Case Management Certification
