About The Position
Signify Health, part of CVS Health®, is seeking a Part Time Physician to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The evaluations are personalized and comprehensive, bringing flexible health services directly to patients' homes.
Requirements
- Active, unrestricted license(s) in coverage area(s).
- Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.
- Physician (MD or DO).

- Multi-state licenses.
Responsibilities
- Conduct in-home health evaluations, lasting approximately 45 minutes.
- Perform medication and medical history reviews.
- Conduct physical evaluations.
- Perform additional diagnostic tests if ordered by the person’s health plan (e.g., diabetic eye exams, spirometry).
- Use an iPad with an intuitive clinical workflow for each evaluation.
- Travel locally and within licensed states as needed.
- Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).
- Conduct virtual visits as needed.
