In-Home Physician (Part Time) - Honolulu, HI
About The Position
Signify Health, part of CVS Health®, is seeking a Part Time Physician to provide In-Home Health Evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The physician will conduct comprehensive and personalized health evaluations, including medication and medical history review, physical evaluations, and potentially diagnostic tests. The evaluations are approximately 45 minutes long, allowing for one-on-one connection and addressing health questions. Physicians will use an iPad with an intuitive clinical workflow and are not required to prescribe medication, order lab tests, or alter current treatment plans. The role contributes to a national network of clinicians focused on improving health outcomes and connecting people to the right care.
Requirements
- Active, unrestricted license(s) in coverage area(s)
- MD or DO degree

Responsibilities
- Conduct In-Home Health Evaluations.
- Engage with individuals in the comfort of their own homes.
- Bridge gaps in care and build trusted relationships.
- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
- Travel locally and within licensed states, as needed.
