In-Home Physician (Full Time) - Norfolk, NE
About The Position
Signify Health, part of CVS Health®, is seeking a Full Time Physician to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The evaluations are personalized and comprehensive, bringing flexible health services directly to patients' homes. The visit, lasting about 45 minutes, includes a medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment plans. The primary workplace is the private residence of health plan members, which can vary in conditions and may include household pets, secondhand smoke, or other airborne allergens.

Responsibilities
- Conduct In-Home Health Evaluations.
- Engage with patients in the comfort of their own homes.
- Bridge gaps in care.
- Build trusted relationships to make people healthier.
- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
- Be flexible to travel locally and within licensed states, as needed.
