About The Position
CVS Health and Signify Health are seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in the comfort of their own homes to bridge gaps in care, build trusted relationships, and provide flexible, comprehensive, and personalized health evaluations. The visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and potentially additional diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by the health plan. Clinicians will use an iPad and an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens. The position is part of a national effort to improve health outcomes by connecting people to the right care for their unique needs.
Requirements
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Able to cover the following primary locations: St. Cloud and surrounding area

Responsibilities
- Provide In-Home Health Evaluations

- Be part of our efforts to visit millions of people nationwide with the goal of providing connections to the right care for people’s unique needs
- Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
- Conduct a medication and medical history review
- Perform a physical evaluation
- Perform additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.) if ordered by the person’s health plan
- Use an iPad and an intuitive clinical workflow for each evaluation
