About The Position

Signify Health, part of CVS Health®, is seeking a Full-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships. The goal is to provide flexible, comprehensive, and personalized health evaluations directly to individuals. The visits, lasting about 45 minutes, allow for one-on-one connection, medication and medical history review, physical evaluation, and potentially diagnostic tests like diabetic eye exams or spirometry. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can be diverse and may contain household pets, secondhand smoke, and other airborne allergens or irritants. Candidates must be able to perform duties comfortably and safely in these environments.

Board certification (required)
Able to cover the following primary locations: St. Lawrence/ Plattsburgh, NY
