About The Position

This role focuses on providing one-to-one patient care in a home setting. Caregivers are expected to support clients with daily living activities, maintain a safe and clean home environment, monitor medications and symptoms, engage in therapeutic activities, and accurately document care provided. Requirements

Completion of a state-approved Home Health Aide program, CNA, or similar trainings
Minimum 2 years of relevant experience.
Valid driver’s license and reliable transportation to reach clients’ homes.
CPR certification (or willingness to obtain within 30 days).
Clear, confident communication and a passion for relationship-centered care.

Responsibilities

Safely assist with transfers, bathing, grooming, toileting, and mobility, always protecting dignity.
Change linens, do light laundry, and maintain a hazard-free, sanitized space.
Provide medication reminders, observe for changes, and document vitals or red-flag signs for the supervising nurse.
Engage in therapeutic activities such as reading aloud, reminiscing, guiding range-of-motion exercises, or accompanying short walks to boost mental and physical wellness.
Record care notes, progress, and concerns in the digital charting system before each shift ends.
