About The Position

Breakthrough Healthcare is urgently seeking a compassionate, dependable Caregiver to provide high-quality, person-centered care in a client’s home. This role requires strong initiative, excellent communication, and a genuine commitment to dignity, safety, and engagement. Full-Time, Part-Time and PRN Positions available. Days and Evenings.

Requirements

• Prior caregiving or home care experience preferred
• Ability to provide hands-on personal care with professionalism and respect
• Strong communication and observational skills
• Dependable, punctual, and compassionate
• Ability to work independently and take initiative

Responsibilities

• Actively engage the client through conversation, activities, and meaningful interaction to promote emotional well-being and quality of life
• Demonstrate initiative by anticipating client needs and responding appropriately without constant direction
• Provide care that is individualized and in alignment with the client’s specific needs, preferences, and care plan
• Maintain proper sanitation and cleanliness of the client’s home, including light housekeeping related to care
• Assist with personal hygiene and total grooming, including: Oral care, Hair care, Nail care, Bathing and toileting assistance
• Prepare and serve nutritious meals according to dietary needs and preferences
• Communicate clearly and professionally with clients, families, and supervisory staff
• Observe, document, and report any changes in condition, safety concerns, or unusual circumstances promptly
• Follow all agency policies, care standards, and safety guidelines
