About The Position

Transitions of Care Pharmacist Specialists are registered pharmacists who assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, they provide support of centralized and decentralized medication-use systems as well as deliver optimal medication therapy to patients with a broad range of disease states. Transitions of Care clinical Pharmacist Specialists proficiently provide direct patient-centered care and integrated pharmacy operational services in a decentralized practice setting with physicians, nurses, and other hospital personnel. The Transitions of Care Specialist will primarily provide focused support for the general medicine/inpatient floors to deliver medication therapy management within the general medicine care area(s) and to ensure pharmaceutical care programs are appropriately integrated throughout the institution. In these clinical roles, The Transitions of Care Specialist partner closely with the BSD Hospitalists in all necessary aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to the patients in their assigned areas. Their focus is going to be mainly on pre-defined high-risk patients with higher rate of readmissions. The Transitions of Care Specialist will perform specific tasks to ensure appropriate admission and discharge MedRec process for these patients.

Requirements

  • Graduate of an ACPE accredited college of pharmacy
  • Doctor of pharmacy or other advanced degree required
  • Completion of an ASHP accredited PGY-1 Pharmacy Residency is highly desired or 3-5 years in an ambulatory, transitions of care, or inpatient setting with transitions of care experience
  • PGY2 Pharmacy Residency in Ambulatory Care or Internal Medicine Highly Preferred

  • Completion of an ASHP accredited PGY-1 Pharmacy Residency
  • PGY2 Pharmacy Residency in Ambulatory Care or Internal Medicine

  • Promote excellent customer service by assisting patients, co-workers, medication staff, and other health care providers

  • Maintains up-to-date knowledge of contemporary hospital practice and clinical service
  • Maintain a specialized evidence-based knowledge of drug and drug therapy by reviewing relevant professional journals and readings
  • Maintain a high professional profile and active membership in relevant professional organizations
