About The Position

Immanuel Pathways PACE® is seeking an IDT Care Coordination Specialist to join their team at Pathways Omaha. This role is crucial for supporting interdisciplinary care planning and ensuring regulatory compliance, aligning with Immanuel’s mission and values. The specialist is responsible for preparing for and facilitating all Interdisciplinary Team (IDT) meetings to ensure regulatory compliance. This includes monitoring and completing all IDT regulatory requirements within specified timeframes, structuring and distributing IDT agendas in advance, and efficiently scheduling and executing care planning sessions. The position also involves supporting and embodying Immanuel’s Mission and CHRIST Promises.

Requirements

  • Bachelor’s degree in healthcare administration, nursing, social work or related field (Equivalent years of experience may substitute for education requirement)
  • Three (3) years’ experience in health care/long-term care operations, care coordination, or program management
  • One (1) year of experience working with the frail or elderly population required, or completion of job specific training related to working with the elderly population must be completed within the first six months of hire
  • Strong project management and organizational skills
  • Proficiency in EPIC electronic health record system
  • Proficiency in Microsoft Teams, Excel, Word
  • Proficiency in Smartsheet
  • Understanding of CMS regulatory timelines and audit expectations
  • Proactive communication
  • Systems thinking abilities
  • Skills in detail orientation
  • Process improvement mindset
  • Ability to manage competing priorities

  • Experience in PACE or managed care

Responsibilities
