About The Position

America’s Home Health Services is seeking a compassionate and organized IDD/Autism Coordinator to oversee and support in-home services for individuals with intellectual and developmental disabilities (IDD) and autism. This role is ideal for a professional who is passionate about person-centered care, experienced in coordinating services, and skilled in working with caregivers, families, and community resources.

Requirements

  • Experience working with Pennsylvania’s Office of Developmental Programs (ODP)
  • Knowledge of waivers
  • Knowledge of Regulations
  • Knowledge of Process
  • Strong experience in multiple software interfaces including but not limited HCSIS and Promise
  • Home Care and/or Home Health experience a MUST HAVE
  • Strong knowledge of person-centered planning and service coordination
  • Excellent communication, organizational, and problem-solving skills
  • Ability to manage multiple cases and priorities effectively
  • Experience supervising or supporting Direct Support Professionals (DSP)
  • Proficiency with documentation

Responsibilities

  • Coordinate and oversee care plans for individuals with IDD and autism receiving in-home services
  • Ensure all services are delivered in accordance with individualized service plans (ISPs) and regulatory requirements
  • Act as the primary point of contact for clients, families, support coordinators, and interdisciplinary teams
  • Conduct client assessments, reassessments, and ongoing monitoring to ensure quality care and satisfaction
  • Provide guidance, training, and support to Direct Support Professionals (DSP) working with IDD/autism clients
  • Assist with recruiting, onboarding, and retaining qualified DSP’s to support program needs
  • Maintain accurate documentation, service records, and compliance with state and company standards
  • Coordinate schedules and ensure appropriate staffing coverage for all assigned cases
  • Participate in care team meetings, case conferences, and community outreach efforts
  • Monitor service outcomes and implement improvements as needed to enhance quality of care
