About The Position

To provide professional social work, education and advocacy to individuals with intellectual disabilities and their families; to locate, coordinate and monitor needed services and supports, to access quality services and supports so that individuals with intellectual disabilities may live self-determined lives, and to be accepted as valuable members of the community.

Requirements

• Bachelors Degree in Psychology, Social Work, Counseling or related field with three years experience working directly with individuals in ID area.
• Valid Pennsylvania Drivers license.
• Basic cell phone, computer and keyboard skills, including familiarity with Microsoft Office (Word, Excel and Outlook) and the internet.
• Successful completion of Acts 73 and / or 169 clearances upon commencing employment, if applicable.

• ID Supports Coordination experience.
• Extensive knowledge of persons with disabilities, knowledge of regulatory agencies and payor policies, as well as community resources.
