About The Position

PCS is seeking an ID/DD Support Coordinator to provide support coordination & case management services to adults with developmental/intellectual disabilities and assure that these individuals receive appropriate services, assisting them in accessing needed medical, psychiatric, educational, residential and other support services.

Requirements

• A Bachelor's degree in Behavioral Sciences or related Human Services field
• Minimum of one-year experience working with the population
• Basic computer skills
• Valid driver's license
• Safe driving record
• Reliable ability to transport self and others to worksites, trainings, and meetings
• Willing to consent to background checks, including checks for barrier crimes, and drug screening

Nice To Haves

• Experience with assessment and service plan development
