About The Position
Provide support coordination & case management services to adults with developmental/intellectual disabilities. Assure that individuals receive appropriate services and assist them in accessing needed medical, psychiatric, educational, residential and other support services.
Requirements
- One-year experience working with the population required.
- Must be computer literate.
- Willing to consent to background checks, including checks for barrier crimes, and drug screening as part of the application process.
Nice To Haves
- Bachelor's degree in human services field preferred.
- Experience with assessment & service plan development desirable.
Responsibilities
