About The Position

ICPR Clinical Case Manager provides case management services to maximize opportunities available to people living in the community who are trying to recover from the catastrophic effects of a serious mental illness. This role requires traveling throughout the St. Louis city and county to meet with clients in their homes to provide crisis-based therapy services, family therapy, and behavioral based interventions for families, youth, and young adults. This is a full-time position (40 hours/week) that requires flexibility for day, afternoon, or evening shifts to meet with clients. Services are provided during daytime hours, but there is also a standard on-call system in place. The team and supervisor provide coverage when the employee is on PTO. There is a maximum caseload of 5-6 clients, with each client seen a minimum of three times per week. A typical day will usually consist of 2-3 therapy sessions, coordination with the case management team, and session note writing. The position requires the candidate to be fully licensed in MO as an LPC or LCSW.

Requirements

  • Must be Fully licensed in MO – LPC or LCSW
  • Master's Degree in Social Work
  • 2-5 years of experience
  • Class D (IL) or Class E (MO) license
  • Ability to travel throughout St. Louis city and county
  • Flexibility for day, afternoon, or evening shifts

Responsibilities

  • Authorizes, coordinates, monitors, and adjusts services with contract providers.
  • Identifies, develops, and maintains good relationships and linkages with a wide range of community resources.
  • Ensures follow through with appointments by assisting clients to keep appointments.
  • Provides collaborative clinical case management services.
  • Provides crisis-based therapy services, family therapy, and behavioral based interventions for families, youth, and young adults.
  • Writes session notes.
  • Coordinates with the case management team.
