About The Position

Case management positions consist of advocating and coordinating services for individuals with intellectual/developmental disabilities.

Requirements

  • Bachelor’s degree in Social Work, Human Services, or a related field preferred.
  • Relevant fields may include Social Work, Psychology, Rehabilitation, Nursing, Counseling, Education, Gerontology, Human Services, or Sociology.
  • Relevant experience may include case management or direct work with individuals with intellectual/developmental disabilities, mental health conditions, or other vulnerable populations, including roles such as CNA, CMA, home health aide, personal care assistant, or similar.
  • Unpaid experience such as internships, volunteering, and caring for a family member may also be considered.
  • Valid driver’s license and auto insurance.
  • Strong organizational skills.
  • Ability to multi-task.
  • Excellent communication skills.
  • Self-motivated.
  • Ability to conduct and facilitate meetings.
  • Ability to collaborate with other team members.
  • Ability to advocate for the needs of participants.
  • Must be able to operate a motor vehicle and have reliable transportation with valid insurance.
  • Must provide verbal and written communication with team members.

Responsibilities

  • Develop person-centered service plans and conduct assessments as required by the program.
  • Conduct monthly face-to-face visits with participants.
  • Assist clients with accessing community resources.
  • Monitor participant progress with waiver services, including health, safety, and welfare in monthly contact notes.
  • Assist with building a network of supports.
  • Assure participant choice and access to community resources.
  • Return phone calls and emails promptly.
  • Complete and submit documentation by required deadlines.
  • Perform other related work as required and assigned.
