About The Position

3HC Home Health and Hospice is seeking a compassionate Hybrid HH & Hospice Nurse, RN to join their team. This role is responsible for administering skilled nursing care to patients requiring intermittent professional nursing service and coordinating all Hospice Services in accordance with the Hospice philosophy, Agency goals, objectives, and policies. 3HC has been providing compassionate care to North Carolinians since 1981, focusing on high-quality, cost-effective care that surpasses industry standards.

Requirements

• Registered Nurse currently licensed to practice nursing in the state of North Carolina.
• Minimum of one-year experience as a professional nurse in an acute care setting.
• Maintain current CPR certification (BCLS).
• Basic computer skills/knowledge and experience in electronic medical records.
• Ability to understand and communicate well with patients, family, staff, physicians, and the public.
• Must be dependable and capable of dealing pleasantly and tactfully with patients, family, staff, providers, and the public.
• Must be competent to perform delegated tasks to facilitate positive patient outcomes as indicated in the patient's individualized plan of care.
• Possess valid Driver's License and reliable transportation.

Nice To Haves

• Previous experience working with terminally ill patients preferred.

Responsibilities

• Administering skilled nursing care to patients requiring intermittent professional nursing service.
• Coordinating all Hospice Services in accordance with the Hospice philosophy, Agency goals, objectives, and policies.
• Managing assigned cases and assisting the office with achieving positive patient outcomes, strategic plan, and budget.
• Providing skilled nursing care as outlined in the physician's plan of treatment.
• Observing and documenting signs and symptoms, and reporting reactions to treatments, drugs, and changes in patient's condition to the physician and other applicable personnel.
• Ensuring ongoing evaluation of patient/family needs and response to care.
• Referring to other services as needed.
• Initiating appropriate preventative and rehabilitative nursing procedures.
• Educating, supervising, consulting, and counseling the patient/family regarding nursing care needs, disease process, self-care techniques, and ethical concerns.
• Assuring the development and implementation of the patient's individualized plan of care and updating as needed.
• Conducting a complete assessment of the patient to identify appropriate care needs, including medical history, physical assessment, and assessment of the support system.
• Interpreting nursing and other services of the agency to the patient and family.
• Planning for discharge from services.
• Completing OASIS at specific points in time.
• Providing comfort to patient/family to facilitate quality of life and death with dignity.
• Completing hospice admissions and educating patients and families on the hospice concept and benefit.
• Planning, coordinating, providing, and directing the delivery of compassionate, quality patient care.
• Actively participating in the coordination of all aspects of the patient's hospice care.
• Providing continuity of care to all hospice patients.
• Assisting the interdisciplinary team in the development and implementation of the patient/family care plan.
• Providing end-of-life care as outlined in the plan of care and policies and procedures.
• Providing ongoing assessment of medicinal and DME needs.
• Participating in QAPI and QI projects as requested.
• Assisting Social Workers with bereavement counseling as requested.
• Continuously evaluating the patient’s condition to provide support and nursing visits individualized to the patient need.
• Notifying attending physicians, staff members, and family members of patient deaths.
• Collaborating with PBM regarding medication changes, administration, and recommendations.
• Documenting services rendered, conferences held, changes in patient's condition, revisions in the nursing care plan, and evaluations of patient care in the medical record electronically.
• Ensuring physician involvement in ongoing patient care.
• Providing regular supervision to Licensed Practical Nurses and paraprofessionals.
• Collaborating with supervisor to meet assigned productivity of 100%.
• Rotating weekday, weekend, and holiday 'on-call' and 'back-up-call'.
• Maintaining regular communication with supervisor.
• Communicating problems of the patient(s) on a timely basis following chain of command.
• Ensuring that 3HC can bill for services appropriately.
• Traveling to other offices to meet the demand of patient needs as requested.
• Staying abreast of overall patient care and improving nursing care skills.
• Participating in office staff meetings, patient care conferences, record audits, utilization reviews, quality improvement activities, and in-services.
• Attending and participating in interdisciplinary team conferences.
• Attending agency mandatory in-services and satisfying CEU requirements.
• Striving to improve nursing care through continuing education.
• Demonstrating an interest in personal and professional growth for self and staff.
• Recommending educational opportunities for staff and encouraging participation.
• Demonstrating a willingness to be cost-effective in the use of agency resources.
• Adhering to 3HC's Personnel Policy and performing other appropriate duties as assigned by supervisor.
