About The Position

The Hunt County Living Room Program Manager position oversees Care Coordinators and Drop Off Coordinators timely intervention in the least restrictive level of care. This role provides comprehensive, trauma-informed, person-centered, recovery and resilience-oriented efforts to help people in crisis gain recovery and stability in the community. The position primarily serves their assigned county and requires excellent organizational and clinical skills to provide continuity of care and crisis response solutions through intersystem and interdepartmental functions and processes. The Program Manager will also be trained as a Care Coordinator and Drop Off Coordinator, providing complex services including screenings, assessments, crisis follow-up, and transitional services to connect individuals with mental illness and/or co-occurring disorders to other services, preventing re-emerging crises and eliminating barriers to treatment. The Program Manager actively facilitates staffing to determine best treatment options and collaborates with internal departments, law enforcement, hospitals, and community agencies. This position provides skills training and case management to staff and individuals at risk for recidivism, hospitalization, and/or homelessness. Travel is required, varying from 20% to 100% per day, covering all counties within the NTBHA Catchment area. This role may require the use of a fleet or personal vehicle for business needs, including field and home visits, offsite meetings, and co-located sites. Transport of individuals is only allowed with a company vehicle and management approval. Availability on non-traditional days and hours, including after-hours cellular availability and some weekends, is required. This position is classified as exempt and requires flexibility in scheduling to meet organizational needs.

Requirements

  • Bachelor's degree in psychology, social work, or a related behavioral health field.
  • One or more years of direct care experience working with individuals with serious mental illness and/or substance use disorders.
  • Experience participating as part of a treatment team that develops and monitors treatment plans for individuals with chronic and serious mental illness.
  • Current knowledge of community resources, providers, and social supports for homeless and indigent populations within the NTBHA service area.
  • Must possess and maintain a valid driver's license.
  • Must maintain a clear background record as required for healthcare organizations under state and federal contracts.
  • Knowledge of rules, regulations, and other guidelines of HHSC.
  • Knowledge of DSM 5 Criteria.
  • Knowledge of the various community resources and providers within the NTBHA service area including (but not limited to) supportive housing and employment, shelters, food banks, social service agencies, charities, medical and behavioral health providers, and other resources for the indigent population.
  • Communicate effectively and use effective interpersonal skills to communicate ideas and complex concepts with individuals from diverse cultural and socio-economic backgrounds through oral and written communication.
  • Constructively and efficiently work with other professionals and staff, community representatives, persons with disabilities, and family members.
  • Foster strong, positive, and effective working relationships with inter-agency and intra-agency team members, encouraging and supporting interaction among various team members across organizational lines.
  • Respect diverse views and approaches, and contribute in maintaining an environment of professionalism, tolerance, civility, and acceptance toward all employees, patients, and visitors.
  • Must be able to work on and have knowledge of a PC (personal computer).
  • Advanced Microsoft Office Suite skills - Outlook, Word, Excel, PowerPoint, etc., with an ability to become familiar with company-specific programs and software.

Nice To Haves

  • Master's degree is preferred.
  • Preference given to experience in psychiatric crisis service settings.
  • Understanding of and experience with HHSC programming and systems preferred, including Texas Resilience and Recovery, CMBHS, and CARE.

Responsibilities

  • Provides skills training and case management to staff and individuals at risk for recidivism, hospitalization, and/or homelessness.
  • Reports to the Director of Hunt Living Room for daily, weekly, and monthly tasks supporting referrals, documentation, and reporting.
  • Maintains data and completes reports by deadlines.
  • Assists and supports the Director with supervision and outreach.
  • Supervises the day-to-day functions of the ELR Program and staff.
  • Reviews monthly staff self-evaluations and provides personalized feedback.
  • Monitors staff schedules.
  • Assists in collecting data for the Director.
  • Trains and supervises assigned staff members in day-to-day operations for all programs represented at their Living Room location.
  • Works autonomously to make decisions consistent with applicable rules, regulations, policies, and procedures.
  • Creates, analyzes, prepares, and maintains records, files, documents, reports, and correspondence.
  • Recognizes problems and identifies and facilitates solutions.
  • Monitors schedules on a daily basis for Hunt Living Room to ensure adequate staff coverage and assigns new tasks as needed.
  • Checks in with staff several times a day to provide assistance, assign tasks, or train if needed.
  • Performs monthly chart reviews and provides feedback for staff growth and development and charting compliance.
  • Maintains program within budgetary guidelines and ensures adequate supplies for work and drop-off.
  • Oversees the designated safety compliance staff and checks to ensure building checklists and required fire drills are completed monthly.
  • Oversees the review and approval of ELR staff timecards in a timely manner by HR deadlines.
  • Fosters teamwork and provides leadership.
  • Generates and manages case referrals and clinician caseloads to ensure required weekly contacts are met and addresses any barriers.
  • Meets at least once a month with each care coordinator for individual supervision and at least once a month as a group supervision to target clinician competency needs and remove individual barriers to effective implementation of job duties.
