About The Position

This position focuses on supporting families connected to Trillium and receiving care at UNC Pediatric outpatient services, CIDD clinics, and identified community practices. The role involves providing emotional, informational, and instrumental support to families to help them acquire desired services, resources, supports, and therapies. The Family Navigator (FN) will also serve as a professional peer resource for families seen by primary care providers and will be linked to a network of UNC and NC-based system family navigators. Key responsibilities include supporting families in identifying needs, liaising with referring providers and community organizations, educating pediatricians and trainees about ADHD, networking with similar programs, assisting in the development of training materials, and tracking/reporting outcomes. Additional duties include conducting needs assessments, communicating needs to clinical teams, building tailored support, initiating contact with families and providers, and enhancing advocacy skills. FNs will also receive and track referrals, document outcomes, maintain clear communication, and participate in team meetings, peer support, and supervision. Documentation will occur in the Epic electronic health record, Microsoft Word, Microsoft Excel, and Outlook email, adhering to HIPAA and patient confidentiality rules. FNs will attend weekly clinical team meetings to report progress, share family information, and advocate for family priorities.

Requirements

  • Support Trillium connected families receiving care at UNC Pediatric outpatient services, CIDD clinics, and identified community practices.
  • Serve as a professional peer resource for families seen by primary care providers (PCPs) in outpatient services and clinics.
  • Be linked to the continually expanding network of UNC and NC based system family navigators via separate yet related projects.
  • Serve as a family system navigator.
  • Work directly with patient families to provide emotional, informational, and instrumental support to acquire desired services, resources, supports, and therapies.
  • Document or communicate in Epic electronic health record, Microsoft Word, Microsoft Excel and Outlook email.
  • Adhere to all HIPAA compliance and patient confidentiality rules.
  • Engage in at least weekly clinical team meetings.

Responsibilities

  • Support families (with emphasis on Eastern NC/Trillium connected families) directly to identify family and patient needs.
  • Work with relevant referring UNC pediatric medical providers.
  • Liaise and develop key partners and organizations in North Carolina.
  • Support the teaching of other UNC pediatricians and pediatric resident physicians, medical students, and LEND trainees about ADHD and common ADHD support needs.
  • Connect and/or network with similar efforts such as Duke Pediatrics program.
  • Interface with other family system navigators from related projects at UNC, Duke, and elsewhere.
  • Assist in the ongoing development of FN training materials.
  • Assist in the tracking and monitoring and reporting of key outcomes.
  • Conduct needs assessment with patients and family to identify needed resources.
  • Communicate identified needs with clinical teams.
  • Build and provide tailored support for clinical teams, patients and families.
  • Initiate and respond to contact from families and community service providers.
  • Help patients, families, and providers increase their knowledge of programs and systems as well as develop their advocacy skills.
  • Receive and track referrals and document outcomes of support provided.
  • Demonstrate accurate, clear, professional, and timely communication.
  • Engage in regularly scheduled team meetings, peer support, and supervision.
  • Document or communicate in Epic electronic health record, Microsoft Word, Microsoft Excel and Outlook email.
  • Adhere to all HIPAA compliance and patient confidentiality rules.
  • Engage in at least weekly clinical team meetings in which they will report back progress, share new information gathered from the family, and advocate on behalf of the family’s self-determined and expressed priorities.
