Excelsior Wellness is seeking an employee to serve as the HUB Access Coordinator, an additional leadership assignment that supports referral operations, client access, and successful connections to services across the organization. This is not a standalone position. The selected employee will maintain their current role while dedicating approximately 15–20 hours per week to HUB Access Coordinator responsibilities. Time allocation may vary based on referral volume, staffing coverage, and operational needs. The HUB Access Coordinator helps ensure every individual expressing interest in services is welcomed, contacted, guided, and connected to the appropriate program. By supporting referral workflow from initial interest through enrollment, this role strengthens access to care, improves communication, reduces delays, and supports a consistent experience for individuals, families, referral sources, and program teams. The HUB Access Coordinator provides day-to-day operational leadership for referral, outreach, intake coordination, client navigation, and enrollment support activities. The role works closely with Care Coordinators, Community Health Workers, and other designated staff responsible for receiving, assigning, tracking, and following up on referrals. The coordinator reports to and works in partnership with the Executive Director of EFM, who oversees HUB referral operations, and communicates referral trends, operational barriers, staffing needs, and performance outcomes.
