The Housing Supportive Service Program (HSSP) is a specialized program that provides intensive field-based community mental health services designed to meet the unique needs of individuals in Permanent Supportive Housing (PSH) who are formerly homeless and have a serious mental health diagnosis. In PSH partnered with Office of Diversion and Reentry (ODR), all residents have some history of justice involvement, and many have a co-occurring substance use disorder. In partnership with the HSSP-ODR team, the Substance Abuse Case Manager performs a variety of tasks to assist clients in in their recovery from substances and in regaining control over their lives. The Substance Abuse Case Manager serves as a primary case manager, provides information and support for clients, and assists clients in gaining admission to inpatient or more intensive substance abuse treatment settings as needed. The Substance Abuse Case Manager will model competence in wellness, recovery, and coping, and is responsible for maintaining the highest quality services and efficiency of operations.
