About The Position

The Housing Supportive Service Program (HSSP) is a specialized program that provides intensive field-based community mental health services designed to meet the unique needs of individuals in Permanent Supportive Housing (PSH) who are formerly homeless and have a serious mental health diagnosis. In PSH partnered with Office of Diversion and Reentry (ODR), all residents have some history of justice involvement, and many have a co-occurring substance use disorder. In partnership with the HSSP-ODR team, the Substance Abuse Case Manager performs a variety of tasks to assist clients in in their recovery from substances and in regaining control over their lives. The Substance Abuse Case Manager serves as a primary case manager, provides information and support for clients, and assists clients in gaining admission to inpatient or more intensive substance abuse treatment settings as needed. The Substance Abuse Case Manager will model competence in wellness, recovery, and coping, and is responsible for maintaining the highest quality services and efficiency of operations.

Requirements

  • High School diploma or GED
  • Certification in substance abuse counseling
  • Current, valid California Drivers License with an acceptable driving record and reliable vehicle
  • Able to transport clients in your vehicle
  • Minimum two (2) years’ experience as a substance abuse counselor and/or case manager, in a licensed and/or accredited substance abuse treatment facility
  • Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers
  • Computer literacy, including knowledge of Microsoft Word, Outlook and Excel
  • Detail oriented with excellent time management, organizational, written, verbal, and computer skills
  • Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision
  • Able to obtain and maintain CPR/First Aid certification

Nice To Haves

  • Associates or Bachelors degree in related field
  • Knowledge of 12-Step organizations, culture, terminology, and literature
  • Certification in CAADE and/or CAADAC
  • Experience leading a 12-Step Facilitation or Treatment Readiness group
  • Experience in a dual-diagnosis setting
  • Working knowledge of criminal justice system
  • Bilingual in English/Spanish

Responsibilities

  • Utilize harm reduction, social, environmental, and motivational, strategies to support clients, in collaboration with licensed clinicians, other counselors, case managers, and support staff.
  • Conduct and document individual counseling sessions with clients at regular intervals determined by individual need.
  • Provide linkage and referrals to outpatient and residential substance abuse treatment and other needed supports.
  • Promote an environment that is strength based to assist clients in meeting their individual goals
  • Assist clients with becoming involved in daily activities, socialization, health maintenance, sense of community and progress in recovery reflective of The People Concern’s model which emphasizes consumer choice, harm reductions, and psycho-social rehabilitation.
  • Work cooperatively with other members of the HSSP-ODR team, including participating in weekly staff meetings, supervision, and training.
  • Document and account for time spent on client contacts and activities in client electronic health record in accordance with Department of Mental Health standards.
  • Maintain a current, thorough knowledge of ongoing developments and new techniques in the fields of substance abuse, mental health and behavioral health community resources and utilize these to provide comprehensive, wrap-around services to clients.
  • Assist in crisis intervention as needed, with support from HSSP-ODR team.
