About The Position

This role is responsible for all aspects of an assigned caseload, including regular client meetings, ensuring clinical files and reports are complete and up-to-date. The position requires a commitment to a client-centered approach, providing services and referrals to address the housing needs of individuals experiencing homelessness. Responsibilities include tracking housing placement trends, orienting clients with the housing group, securing entitlements, maintaining correspondence related to housing interviews and vacancies, and assisting clients with coordinating moves with landlords and other agencies. The role also involves serving as a liaison with DHS and other providers to facilitate the housing process, providing housing retention services, participating in interdisciplinary team meetings and case conferences, and providing emergency first aid/CPR assistance when needed. Related duties are also assigned.

Requirements

  • High School Diploma/GED required.
  • Significant experience working with individuals experiencing homelessness.
  • Significant experience with securing entitlements and housing.
  • Demonstrated writing skills.
  • Demonstrated communication skills.
  • Demonstrated group facilitation skills.
  • Proficiency in MS Office, including Word and Excel.
  • CPR training certification or willingness to complete certification training in CPR.

Nice To Haves

  • Bachelor's Degree preferred.
  • Knowledge of mental illness and substance abuse helpful.
  • Bilingual abilities a plus.

Responsibilities

  • Assist clients to coordinate their move with landlords and other internal personal and external agencies.
  • Serve as a liaison with DHS and other providers to facilitate the housing process for clients.
  • Provide housing retention services once clients have been placed.
  • Participate in interdisciplinary team meetings and case conferences.
  • Provide emergency first aid/CPR assistance when needed.
  • Perform related duties as assigned.
