About The Position

This role is responsible for managing a caseload, ensuring client files are complete, and engaging individuals in a client-centered approach to address housing needs. The specialist will provide services and referrals, track housing placements, orient clients to the housing group, secure entitlements, maintain correspondence, and assist clients with coordinating moves. A key aspect of the role involves serving as a liaison with DHS and other providers to facilitate the housing process and providing housing retention services post-placement. The position also includes participation in interdisciplinary team meetings, case conferences, and providing emergency first aid/CPR assistance when needed. Related duties are also assigned.

Requirements

  • High School Diploma/GED required.
  • Demonstrated writing, communication and group facilitation skills.
  • Proficiency in MS Office, including Word and Excel.
  • CPR training certification or willingness to complete certification training in CPR.

Nice To Haves

  • Bachelor's Degree preferred.
  • Significant experience working with individuals experiencing homelessness.
  • Experience with securing entitlements and housing.
  • Knowledge of mental illness and substance abuse helpful.
  • Bilingual abilities a plus.
  • Vaccination preferred but not required.

Responsibilities

  • Manage all aspects of assigned caseload, including meeting with clients regularly.
  • Ensure all clinical files and reports are complete and up to date.
  • Engage individuals in a client-centered approach.
  • Provide services and referrals to address varied housing needs of individuals experiencing homelessness.
  • Track trends in housing placements.
  • Orient clients with the housing group.
  • Secure entitlements.
  • Maintain correspondence relating to housing interviews and vacancies.
  • Assist clients to coordinate their move with landlords and other internal personal and external agencies.
  • Serve as a liaison with DHS and other providers to facilitate the housing process for clients.
  • Provide housing retention services once clients have been placed.
  • Participate in interdisciplinary team meetings and case conferences.
  • Provide emergency first aid/CPR assistance when needed.
