The Housing Specialist (Supportive Services) main purpose is to provide in-home and community support to individuals attempting to find or maintain independent housing in the community. This includes providing training in the independent living skills needed to maintain a residence. This position has been identified as a safety sensitive position that includes tasks or duties (i.e., driving) that the employer in good faith believes could affect the safety, health or reputation of the Agency, participants, the employee performing the task or others. In all duties listed in the job description, the Housing Specialist (Supportive Services) is responsible for support services to an assigned caseload and documenting services provided. Requirements

  • HS diploma or GED with relevant or housing experience.
  • Must be 21 years of age or older.
  • Possess Arizona driver’s license with acceptable driving record including but not limited to no serious traffic violations (ie. Reckless Driving; at fault accident; felony speeding/20 mph or over) or greater than 2 traffic violations in prior 39 months.
  • Certification in first aid, CPR, and de-escalation training; if candidate does not possess active certification agency will provide training.
  • Obtain Tuberculosis screening test upon hire to provide evidence that the individual is free from infectious tuberculosis prior to providing services and additional TB testing or screening questionnaires will be performed annually.
  • Possess the physical, mental, and cognitive ability to perform specific clinical and/or tasks required by the position without posing a direct threat to the safety and security of patients.
  • Ability to type and working knowledge/familiarity with computers and electronic clinical record keeping.
  • Must obtain and maintain a fingerprint clearance card and pass a background check.
  • Demonstrate good written and verbal communication skills.
  • Able to positively interact and develop rapport with mentally ill participants and their families, professional and support staff, and various levels of staff from community agencies.
  • Able to maintain a calm, non-defensive, supportive attitude during crisis or potential crisis situations.
  • Long periods of sitting, ability to bend, lift and carry up to 50 pounds.
  • Able to operate phone, Fax, copier, and a computer.
  • Must be fully ambulatory and able to assist participants.
  • Must be able to operate an Agency vehicle

Nice To Haves

  • Combination of education and experience to meet qualifications for Behavioral Health Tech (BHT).
  • Experience working in Integrated Healthcare, Behavioral Health, Residential or Housing.
  • Advanced knowledge of cultural diversity and sensitivity.
  • Bilingual – English/Spanish

Responsibilities

  • Provide in-home and community support services in a Trauma Informed Recovery Focused manner as outlined in individual’s treatment plan.
  • Support patients with housing location services including application process and searches.
  • Provide education and assistance in independent living skills such as personal care, organization, food preparation, clothing care, money management and general home cleanliness.
  • Provide referrals and education regarding community resources and socializing within the community.
  • Act as subject matter expert related to housing such as location services, application process, etc.
  • Coordination of care with homeless outreach team.
  • Participate in LCEH meeting
  • Completes clinical documentation on individuals and accompanying billing documentation daily.
  • Provide patients with medication education.
  • Provide safe, timely transportation of clients to and from various appointments as assigned.
  • Perform other duties as assigned.
