About The Position

At Easterseals PORT Health (ESPH), our mission is rooted in empowering individuals and strengthening communities. We are seeking a compassionate and dedicated Housing Specialist / Qualified Professional to join our Assertive Community Treatment (ACT) team. In this role, you will support individuals living with severe and persistent mental illness in securing and maintaining safe, stable housing while building the skills needed for long-term community success. Requirements

  • Strong commitment to recovery-oriented, person-centered care
  • Experience working with individuals with severe and persistent mental illness
  • Knowledge of community housing resources
  • Ability to build trusting, respectful relationships that honor client choice and rights
  • Bachelor’s degree in a human services or related field
  • Qualified Professional (QP) status or eligibility
  • Valid North Carolina driver’s license with reliable transportation
  • Willingness to work primarily in community settings (homes, shelters, hospitals, streets, jails)
  • Ability to participate in an after‑hours on‑call rotation
  • Comfort using electronic health records and standard office technology
  • Ability to sit, type, and perform repetitive motions; speak clearly and document legibly

Nice To Haves

  • Experience in ACT, TCLI, or community‑based services preferred

Responsibilities

  • Providing case management for an assigned group of individuals, coordinating care with the Individualized Treatment Team (ITT)
  • Developing, implementing, reviewing, and revising individualized treatment goals and service plans
  • Delivering rehabilitative services, including: Housing and tenancy‑sustaining supports, Money management and entitlement assistance, Activities of daily living (ADLs), Health, wellness, recovery, and community integration
  • Building and maintaining relationships with housing stakeholders, including: Housing Authorities, Permanent supportive housing providers, Rapid re‑housing programs, HUD‑VASH, NC Housing Finance Agency, Local landlords
  • Coordinating communication with guardians, care coordinators, landlords, and community agencies
  • Providing individual and/or group supportive therapy and symptom management interventions
  • Participating in comprehensive assessments including psychiatric history, substance use, physical health, social functioning, and housing needs
  • Supporting individuals during crises and participating in an on‑call rotation
  • Educating and supporting families while advocating for client rights and preferences
  • Documenting services and progress accurately and timely within the electronic health record
  • Participating in daily team meetings, treatment planning, and organizational reviews
  • Providing substance use treatment and recovery-oriented supports as needed
