About The Position
Liberty Healthcare Corporation is hiring for multiple Housing Specialist positions to support the California Conditional Release Program. This program exclusively serves formerly incarcerated adults transitioning into the community, with community safety as the top priority. The program aims to support clients in living healthy and productive lives by focusing on community safety, preventing recidivism, transitioning clients from hospital treatment to the community, securing housing and community supports, proactively addressing client needs and risk factors, and providing behavioral health services, case management, 24/7 monitoring, and transportation.
Requirements
- Must reside in California
Responsibilities
- Researching rental housing markets
- Creating lists of viable housing options for program clients
- Evaluating housing options for alignment with client conditions of release
- Developing relationships with landlords and property managers
- Collaborating with Liberty’s Housing Coordinator, Regional Coordinators and Case Workers
