GENERAL RESPONSIBILITIES Provides subject matter expertise and operational support of case management staff to ensure effectiveness and consistency of program specific procedures, administrative procedures, and adherence to corporate policies. Coordinates and provides supportive services including but not limited to applications, outreach, home visits, social service referrals, crisis intervention, case management, client advocacy and related services to individuals and families assigned to the specific housing program.

Requirements

  • Bachelor's degree in social work or related field or four years of equivalent experience
  • Two years' experience in supportive housing case management
  • Clearance through the Idaho Department of Health and Welfare Criminal History background check.
  • Experience working with underserved populations; knowledge, commitment, and sensitivity to the needs of people with housing insecurity, substance misuse, mental health challenges or other challenging populations
  • A working knowledge of service area social service resources for those patients with behavioral health issues (city, county, state and federal).
  • Ability to work independently.
  • Excellent communication and customer service skills.

Nice To Haves

  • Master's degree in social work or related field
  • Experience working in a multidisciplinary setting.
