About The Position

The Joyce Housing Coordinator supports Cascade AIDS Project’s (CAP) brand, mission, and culture through CAPNW's permanent supportive housing partnership at The Joyce in downtown Portland. CAPNW works alongside Community Partners for Affordable Housing (CPAH), Native American Rehabilitation Association of the NW (NARA NW), Geater New Hope, and Pilina Property Management to provide subsidized housing and intensive supportive services to residents of the building. The coordinator works with onsite partners and CAPNW staff and management to provide high-quality, coordinated, strength-based social services consistent with the agency’s mission. The position coordinates participant referrals and eligibility verification; provides intensive onsite housing case management, including services in residents’ units, goal planning, information and referral, advocacy, tenant education, and eviction prevention; and provides responsive, on-the-spot case management and crisis support to a high-acuity caseload of about 20 households of residents living with HIV at The Joyce who have experienced chronic homelessness and may have co-occurring mental health and/or substance use needs. This is a role Full-Time (32 hours a week) working at The Joyce Hotel located in the West End area of Portland, OR. The schedule for this role is Monday-Thursday 8:30am-5:00pm Starting pay is $54,588.35 a year. The Joyce Housing Coordinator’s pay increases with tenure and the top of the pay scale is $62,836.59. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.

Requirements

  • Bachelor’s degree in a human/social services field (social work, public or community health, psychology) or related field, or two years’ experience relevant to the position
  • Prior experience providing intensive case management or similar services, with knowledge of care coordination, client advocacy, crisis response, and community service navigation
  • Prior experience working with ethnic, racial, economic, and sexually diverse populations and people experiencing chronic homelessness, mental illness, and/or substance use
  • Demonstrated computer proficiency using Microsoft Office software (Word, Excel, Outlook) and willingness to learn new software
  • Excellent written and oral communication skills
  • Ability to engage in equity and inclusion initiatives and discussions in the workplace, including but not limited to engaging in training workshops, team and department meetings, and critical discussions about how CAP can better support the communities we serve

Responsibilities

  • Coordinate participant referrals, eligibility review and verification, applications, leasing processes, and new-tenant orientation with CAP staff and onsite partners.
  • Provide intensive, strengths-based onsite housing case management, including services in residents’ units, advocacy, and eviction prevention that promote client self-determination, independence, and housing stability.
  • Carry an active, high-acuity caseload of about 20 households of residents living with HIV, all located at The Joyce, and assist with housing and goal planning, lease compliance, advocacy, mediation, information and referral, and connection to legal services.
  • Maintain scheduled onsite office hours and provide low-barrier, responsive case management for walk-in, time-sensitive, and emergent resident needs, including assessment, immediate support, de-escalation, referral, and follow-through.
  • Complete comprehensive housing assessments, individualized housing plans, annual goals, service linkages, and advocacy for and with clients, and monitor progress through consistent follow-up.
  • Collaborate with medical case managers, medical providers, Resident Services, property management, and other providers involved in client care to coordinate comprehensive services with a focus on housing stability.
