Host Home Supervisor (Contract Monitor) - Eastern Iowa Region
About The Position
LSI's Services for People with Disabilities is seeking a dedicated, energetic full-time supervisor to work as a Contract Monitor for their Host Home Services in the Eastern Iowa regions. This position will be centralized with homes in the Cedar Rapids, Cedar Valley, Davenport, or Dubuque areas. The role offers some work-from-home flexibility and involves travel within a specified service region, which includes numerous counties in Eastern Iowa. Travel may extend beyond these counties to cover service needs and meetings. The Iowans served by this program are seeking greater confidence and independence, and the role's purpose is to empower them toward success.
Requirements
- HS Diploma/GED and five years of related experience OR Associates Degree and three years of related experience OR Bachelor's Degree and one year of related experience
- Valid driver's license
- Auto insurance
- Good driving record
- Ability to pass an extensive background check including checks of the Dependent Adult and Child Abuse registries, Sex Offender registry and a criminal history check
Nice To Haves
- Bachelor's Degree in a human services related field and one year of related experience
- Years of related experience in a supervisory or leadership role
Responsibilities
- Acting as the main contact for interdisciplinary team members and for individuals served in the host home program
- Working directly with assigned host homes to provide support, assistance, and instruction in areas which facilitate personal growth and independence
- Facilitating and participating in interviews and evaluations related to the contracting process for independent providers
- Overseeing specified Host Homes in the Eastern Iowa service area
- Overseeing the quality of services and communicating with interdisciplinary team members
- Overseeing contractual compliance and ensuring that assigned independent providers are providing quality care to individuals supported as well as meeting contractual requirements
