About The Position
Sample Supports is seeking an energetic and professional Host Home Provider to offer a home to a female individual in her 30s with intellectual disabilities, medical needs, and mental health needs. This role involves providing in-home placement and support within the community, enabling the individual to live as independently as possible. The position requires supporting the individual with behavioral, mental health, medical, and daily living needs, including social and emotional regulation and participation in appointments. Experience with complex medical needs and monitoring for infections or behavioral escalation is ideal.
Requirements
- Must be at least 21 years old.
- Must have a GED or diploma.
- Must have dependable and reliable transportation with the ability to safely transport.
- Must have a valid driver's license.
- Must be able to pass background check.
- Must have an appropriate living environment with at least one available bedroom.
- Must be competent and willing to perform Non-Violent Crisis Intervention physical managements as trained.

- Maintain open communication with individuals guardians.
- Fiscally responsible.
- CNA, LPN, or RN.
Responsibilities
- Provide personal assistance, social and emotional support, and other personal care to adults with intellectual and developmental disabilities in a home and community environment.
- Support with activities of daily living including shopping and meal preparation, personal care/hygiene routines, and general in-home support tasks as needed.
- Engage in community activities based on the needs of the individual, potentially creating a unique activity plan together.
- Provide transportation services as needed, including to and from scheduled activities and appointments.
- Complete documentation for services provided.
- Administer medication and provide support as needed.
- Report any suspected maltreatment, abuse, neglect, or exploitation of an at-risk individual (M/A/N/E).
