About The Position

Continuum of Colorado is seeking compassionate and committed Host Home Providers in the Aurora and Denver Metro area to support adults with intellectual and developmental disabilities in a safe, caring, and supportive home environment. As a contracted provider, you’ll open your home and your heart to help someone thrive in a community-based setting. For this opportunity, we are especially interested in providers who are comfortable supporting individuals with higher behavioral and/or medical needs. The ideal provider is patient, dependable, calm under pressure, and confident in providing consistent support while maintaining a person-centered approach.

Requirements

  • Must be 18 years of age or older
  • Pass a criminal background check
  • Complete all state-required trainings and certifications
  • Maintain liability insurance as outlined in contract
  • Comply with all state and federal regulations for residential services

Nice To Haves

  • Experience in caregiving and/or Host Home settings supporting individuals with I/DD
  • Demonstrated skill in behavioral de-escalation, with an emphasis on consistency and structure
  • Experience with safe physical intervention techniques for high-acuity behavioral situations
  • Ability to maintain a stable, long-term placement
  • Offer wheelchair-accessible homes
  • Capacity to support individuals requiring intensive behavioral or medical care

Responsibilities

  • Offer individualized support to a person with a developmental disability living in your home.
  • Assisting with daily living tasks such as personal care, hygiene, meal preparation, and medication management.
  • Providing behavioral support and responding calmly and consistently to challenging behaviors.
  • Supporting individuals with higher medical or health-related needs and following established care and safety plans.
  • Recognizing and communicating changes in an individual's health, behavior, or overall well-being.
  • Encouraging community involvement, recreation, independence, and life skills development.
  • Providing transportation to appointments, activities, and community outings.
  • Documenting supports and participating in monitoring, training, and team meetings.
  • Working collaboratively with family members, case managers, and other members of the individual's support team.
  • Creating a stable, respectful, and person-centered home environment focused on safety, independence, and quality of life.
