About The Position

Sample Supports is seeking compassionate Host Home Providers to share their homes with adults with disabilities. This unique career offers flexibility and the opportunity to give back to the community by providing care to adults in need. Specifically, we are looking for providers with a wheelchair-accessible home to support an individual who uses a wheelchair. Support needs include assistance with safe transfers, mobility support, and daily living activities such as personal care, meal preparation, and community access. The ideal provider is patient, attentive to safety, and comfortable supporting physical needs while promoting independence and dignity. While experience with wheelchair users or transfer techniques is preferred, training and ongoing support will be provided. Sample Supports will match an individual in services with you based on the compatibility of you, your family, and your home environment. We welcome Host Home Providers of all experience levels that have at least one private bedroom available in their owned or rented home. People who have experience with disabilities and/or caring for adults that are aging either in their own families or professionally fit this role organically.

Requirements

  • Must be at least 18 years old
  • Must be able to lift at least 75 pounds
  • Must have dependable and reliable transportation with the ability to safely transport
  • Must have a valid driver's license
  • Must meet approved motor vehicle record standards for providing transportation for individuals in services
  • Must be able to pass Child/Adult Protection Services check
  • Must complete a criminal background check
  • Must have appropriate living environment with at least one private bedroom available for placements
  • Must be willing to perform Non-Violent Crisis Intervention physical managements as trained

Responsibilities

  • Provide personal assistance, social and emotional support, and other personal care to adults with intellectual and developmental disabilities in a home and community environment
  • Support with activities of daily living including shopping and meal preparation, personal care/hygiene routines, and general in-home support tasks as needed
  • Community activities based on the needs of the individual
  • Transportation services as needed - may include transportation to/from scheduled activities and appointments
  • Complete documentation for services provided using electronic systems
  • Medication administration and support as needed
