About The Position

The Primary Care Department at Albany Medical Center is seeking a part-time Hospitalist Nurse Practitioner or Physician Assistant to join our busy and collaborative primary care Hospitalist team. This position offers the opportunity to be at the center of inpatient care, guiding patients through every step of their hospital journey. Requirements

  • License and active registration as a Registered Nurse, Nurse Practitioner or Physician Assistant in New York State
  • Master's Degree from an accredited Nurse Practitioner program or completion of an accredited Physician Assistant program
  • Familiarity working in electronic medical records.
  • Collaborate effectively across various departments to ensure cohesive, patient-centered care and a positive work environment
  • Maintain HIPAA and patient confidentiality
  • Advanced Trauma Life Support and Basic Life Support certification required
  • Must be able to perform the essential functions of the position in a clinical environment, including, but not limited to, standing, walking, lifting, bending, kneeling, and reaching. Requires sufficient visual, auditory, and manual dexterity abilities to assess patients, provide direct patient care, operate medical equipment, and complete documentation.

Responsibilities

  • Provide comprehensive inpatient care for acutely and chronically ill hospitalized patients
  • Admit, manage, and discharge patients, including completing discharge summaries and care plans
  • Conduct daily patient rounds and adjust treatment based on clinical progress
  • Perform history & physical exams, develop diagnoses, and initiate treatment plans
  • Collaborate closely with the attending physician and other health care providers to provide high-quality patient care
  • Serve as a primary point of communication for patients, families, and care team
  • Educate patients and families on conditions, treatments, and discharge plans in a clear and compassionate manner
  • Document patient encounters and treatment plans accurately in the electronic health record
  • Participate in quality improvement and practice efficiency initiatives
