Requirements

  • Must be at least 18 years of age.
  • A current and valid driver's license is required.

Responsibilities

  • Communicates transportation needs to the Supervisor in a timely manner to ensure individuals receive the most efficient response and can return to their normal routines.
  • Ensures safe transportation by securing wheelchairs and residents in vehicles and supervising during transport.
  • Familiarizes self with doctors’ offices, clinics, and hospital environments.
  • Gathers relevant information about the individual prior to scheduled trips to provide accurate details to professional staff.
  • Arrives on time for all appointments and shares any pertinent resident information with medical personnel.
  • Develops and maintains professional rapport with staff at doctors’ offices, clinics, and hospitals.
  • Monitors residents’ needs for off-campus appointments (e.g., clothing changes, personal items, lunches, etc.).
  • Obtains signed consultation sheets as required.
  • Verbally reports any concerns about medical or professional treatment to the Health Services Supervisor or appropriate supervisory staff.
  • Signs residents in and out for hospital trips or appointments and documents accordingly on the behavior log in their living area.
  • Performs duties of a Habilitation Worker when not scheduled for off-campus trips.
  • Escorts residents using Merakey Allegheny Valley School vehicles or emergency vehicles to doctors’ offices, health centers, or emergency rooms.
  • Performs any related work as required.
