About The Position

Per diem Social Worker - provides psychosocial assessments, crisis intervention, discharge planning, and care coordination for patients across multiple hospital settings, including the Emergency Department, Medical/Surgical units, Birth Center, Intensive Care Unit (ICU), and Psychiatric Medical Unit (PMU). Flexible scheduling!

Requirements

• LLMSW in Michigan
• At least 2-3 years of experience in a hospital or other relevant healthcare field
• Strong psychosocial assessment and critical thinking skills
• Crisis intervention and problem-solving abilities
• Discharge planning and care coordination expertise
• Knowledge of community resources and support services
• Excellent communication and interpersonal skills
• Ability to build rapport with patients, families, and interdisciplinary teams
• Ability to manage competing priorities in a fast-paced environment
• Sound clinical judgment and decision-making skills
• Knowledge of healthcare regulations, patient rights, and mandated reporting requirements
• Highly adaptable and comfortable working in new and changing environments
• Flexible and willing to cover a variety of patient populations and care settings
• Quick learner who can rapidly assess situations and adjust priorities
• Comfortable with ambiguity and unfamiliar situations
• Self-directed and able to work independently with minimal supervision
• Resilient and calm under pressure
• Team-oriented with a collaborative approach to patient care
