About The Position

The Hospital Social Worker is an exciting opportunity at MultiCare, responsible for cross continuum care coordination, psychosocial assessments, and psychosocial interventions. In this role you will identify opportunities to continually improve patient care, services and population health. Requirements

  • Master’s degree in Social Work
  • Licensed in the state of Washington
  • Licensed Independent Clinical Social Worker (LICSW); OR Social Worker Associate Independent Clinical (LSWAIC) with a commitment to obtain their LICSW within four (4) years
  • Minimum three (3) years Hospital or equivalent experience
  • Strong background in mental health and crisis intervention.
  • Ability to remain calm and compassionate in high-stress situations.
  • Skilled in independent decision-making and teamwork.

Responsibilities

  • Perform cross continuum care activities, which may include clinic, provider practice, urgent care, phone care agency, skilled care, or other sites of care, as appropriate
  • Provide customer service to the patient, family, physician/APP, and care team members to include discharge milestones and progression to next level of care
  • Model and orchestrate consistent, professional communications and maintain consistent and proactive communication patterns with colleagues and staff
  • Delegate responsibilities appropriately but remains accountable for outcomes
  • Ensure that the patient and family experiences medically appropriate transitions of care
  • Conduct mental health, suicide risk, and substance use assessments in a dynamic ER setting.
  • Collaborate with Crisis Responders, psychiatric facilities, detox centers, and homeless shelters to ensure safe discharges for patients.
  • Provide resources and emotional support to patients and families during traumatic situations.
  • Problem-solve complex discharge plans and connect patients to community resources.
