About The Position

Provides medication management services in coordination with other health care professionals to deliver safe, appropriate, and cost-effective medication therapy to patients. Responsibilities may include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Participates in collaborative practice and/or protocol-driven practice as appropriate. Additionally, pharmacists are active members in quality improvement, research activities, and education as described in the job specific competencies.

Requirements

Hospital Pharmacist experience
Ability to provide medication management services
Ability to coordinate with other health care professionals
Ability to deliver safe, appropriate, and cost-effective medication therapy
Ability to assess medication therapy regimens
Ability to perform medication therapy management
Ability to verify/enter medication orders
Ability to distribute medications
Ability to monitor medication therapy and consult
Ability to document medication therapy plans
Ability to provide medication use education and pharmaceutical care services
Ability to participate in collaborative practice and/or protocol-driven practice
Ability to participate in quality improvement, research activities, and education
