About The Position

PharMerica is hiring a Pharmacist to work in a fully remote setting, serving Critical Access Hospitals (CAH) and Behavioral Health facilities. This role focuses on remote order verification, clinical review, and oversight of medication use, ensuring safe and compliant pharmaceutical practices across multiple hospital locations. The position requires strong clinical judgment, independent decision-making, and the ability to work effectively with limited supervision.

Requirements

  • Bachelor’s Degree in Pharmacy, Pharm D preferred
  • One year experience in long term care desired
  • Current and valid pharmacist license in the state of practice
  • Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
  • Proficiency in electronic medical records and pharmacy information systems
  • Strong clinical judgement, and effective written and verbal communication
  • Active licenses: Texas (required), New Mexico (required)
  • Willingness to obtain additional state licenses (required)
  • Minimum 2+ years inpatient hospital experience preferred
  • Strong clinical knowledge and independent decision-making ability

Responsibilities

  • Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
  • Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes and optimizations.
  • Maintain up-to-date patient/resident profiles, including medical history, lab results, and current medications (prescription, OTC, and home remedies).
  • Ensure laboratory tests required for monitoring high-risk medications are appropriately ordered, reviewed, and communicated to the healthcare team.
  • Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
  • Document all communications and clinical interventions in the patient’s medical record, including interactions with families and caregivers.
  • Implement and enforce policies and procedures for the safe handling, dispensing, and clinical oversight/management of medications.
  • Ensure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management.
  • Report medication-related errors and adverse drug events to the Pharmacy Manager as required.
  • Educate and support clinical team and healthcare agency staff on safe and effective medication handling procedures.
  • Perform other pharmacy-related tasks and responsibilities as assigned.
  • Review and verify medication orders in real-time, ensuring appropriate indication, correct dose, route, frequency, renal/hepatic adjustments, allergy and interaction screening, and releasing verified orders to ADCs (Omnicell/Pyxis) for nursing access.
  • Perform prospective and retrospective clinical order review, providing recommendations for antibiotic stewardship (IDSA-aligned), renal dosing adjustments, and high-risk medications (anticoagulants, opioids, insulin, vancomycin AUC).
  • Communicate directly with providers and nursing staff.
  • Monitor medication administration trends and discrepancies.
  • Support nursing workflow, including timing of doses and missing medication resolution.
  • Provide guidance on IV compatibility, infusion rates, and administration safety.
  • Review narcotic usage patterns and discrepancies.
  • Support diversion monitoring efforts (DEA expectations).
  • Ensure valid order presence for all controlled substance dispensing and that documentation aligns with administration records.
  • Maintain compliance with CMS Conditions of Participation (CoPs), Joint Commission Medication Management (MM standards), and Texas State Board of Pharmacy (TSBP).
  • Support documentation and audit readiness (72-hour retrieval standard).
