About The Position

DeCoach Recovery Centre is seeking a Hospital Liaison for the Dayton area. This role is the primary point of contact between DeCoach Recovery Centre and local hospitals, ensuring smooth transitions of care for patients needing addiction treatment. The liaison will build and maintain relationships with healthcare professionals, promote DeCoach’s services, and coordinate patient referrals from hospitals to DeCoach’s treatment programs. This position works with the admissions team under the Chief Marketing Officer (CMO) to ensure efficient, effective, and patient-centered outreach and referral processes.

Requirements

Minimum of 2 years in a healthcare liaison, case management, or related role, preferably within addiction treatment, hospitals, or behavioral health services.
Strong interpersonal and communication skills, with the ability to build relationships, advocate for patients, and collaborate effectively with team members.
Familiarity with addiction treatment options, including medication-assisted treatment, detox, and recovery programs.

Nice To Haves

Bachelor’s degree in communication, social work, psychology, healthcare administration, or a related field preferred.
Certified Chemical Dependency Counselor (CDCA) or equivalent certification preferred but not required.

Responsibilities

Develop and maintain strong, collaborative relationships with hospital staff, including physicians, nurses, case managers, and discharge planners.
Facilitate the referral process by coordinating with hospital teams to identify patients in need of addiction treatment and ensuring timely transitions to DeCoach Recovery Centre.
Work closely with the admissions team and other outreach representatives to ensure a cohesive approach to patient care and referral management.
Provide education to hospital staff about DeCoach’s comprehensive treatment services, including our medication-assisted treatment programs, residential care, outpatient services, and recovery housing.
Serve as an advocate for patients, ensuring they receive appropriate care and support as they transition from the hospital to DeCoach’s treatment programs.
Maintain communication with hospital staff and patients post-discharge to monitor progress and address any ongoing needs or concerns.
Accurately document all interactions, referrals, and follow-up activities in accordance with DeCoach’s policies and procedures.
Represent DeCoach at hospital and community events, strengthening our presence and visibility in the healthcare community.
