About The Position

The Hospital Liaison serves as a key point of contact between the hospital, patients, families, and community resources, supporting program objectives through direct client engagement, coordination of services, and collaboration with healthcare providers. In this role, the Hospital Liaison conducts comprehensive interviews with mothers and families in a nonjudgmental, empathetic, and positive manner while promoting initiatives such as exclusive breastfeeding and access to social services. The position requires accurate data collection and documentation using a Windows-based database system, as well as maintaining thorough records of all client interactions and services provided. The Hospital Liaison builds and sustains strong relationships with hospital providers and works collaboratively across departments to support patient care, facilitate hospital visits, and assist with community outreach efforts. Additionally, the role includes helping to develop and maintain a community resource directory, attending staff meetings and trainings, serving as a single point of contact for post-acute providers, and performing other duties as assigned to enhance patient outcomes.

Requirements

  • Bachelor's degree in public health, social work, nursing, healthcare administration, or a related field, or an equivalent combination of education and experience.
  • At least one to three years of experience in a healthcare, hospital, community outreach, or social services environment.
  • Experience conducting client interviews.
  • Experience completing assessments.
  • Experience working collaboratively with multidisciplinary teams.
  • Strong interpersonal skills.
  • Strong communication skills.
  • Strong organizational skills.
  • Ability to maintain confidentiality.
  • Ability to handle sensitive information with professionalism.
  • Proficient in Microsoft Office.
  • Comfortable using database systems or electronic health records.
