About The Position

Centerstone is a leading nonprofit behavioral health system dedicated to providing life-changing care to individuals of all ages across various settings. As a trauma-informed organization, Centerstone fosters a culture of understanding, recognizing, and responding to trauma to build resilience among clients, staff, and the community. This role is responsible for engaging hospitalized patients nearing discharge to ensure they are seamlessly connected with follow-up care at Centerstone. The Hospital Liaison will collaborate with local hospital staff to strengthen partnerships and facilitate the transition of patients to outpatient services. This position requires flexibility and availability to local hospitals via phone or in person, and may involve assisting the Health Link team with care coordination calls. The liaison will also be accessible to their Team Leader and other Centerstone staff via Skype during work hours when not in the field.

Requirements

  • Bachelor Degree required in psychology, social work or related field preferred.
  • Two years of experience working with individuals with serious mental illness preferred.
  • Effectively communicate via written, verbal, in person and virtual methods.
  • Excellent skills with Excel.
  • Exceptional organization skills.
  • Ability to coordinate multiple tasks.

Responsibilities

  • Coordinate with area hospitals to ensure patients discharging to Centerstone are linked to outpatient follow up care.
  • Ensure that all documentation is completed in the Electronic Health Record.
  • Ensure that appropriate follow up with clients is occurring.
  • Coordinate with other Centerstone staff to ensure clients have a smooth transition from inpatient to outpatient services.
  • Document and maintain data tracking in an excel file.
  • Compliance with annual KPI evaluations.
  • Other duties as assigned.
