Requirements

  • High School Diploma / GED or Associates Degree with a combination of eight (8) years’ direct care experience, required.
  • Must have a Level 1 fingerprint clearance card issued by Arizona Department of Public Safety of eligible to receive within eight (8) weeks of hire.
  • Must possess a valid AZ Driver’s License and meet Copa’s driving requirements.

Nice To Haves

  • Bachelor’s degree in behavioral health or related field and three (3) years direct care experience, preferred.
  • Previous experience in behavioral health or primary care with duties of case management and care coordination, preferred.
  • Demonstrated experience and knowledge in discharge planning and readmission prevention processes, preferred.

Responsibilities

  • Under the oversight of the Copa Clinical Director and Director of Medical Management, implements the Copa Hospital Readmission and Discharge Coordination protocol.
  • Collaborates with the Inpatient and Outpatient Care Team, assess individual needs to develop a plan with member that promotes long-term health and reduces the likelihood of re-admission. Serves as an added resource to the assigned primary case manager to facilitate individual engagement and follow-up in care, understand and implement post-discharge/post-visit medical orders and to coordinate more specialized post-discharge/post-visit placements (including skilled nursing centers, home health, and residential settings).
  • Facilitate communication and coordination with the Inpatient Team, RBHA, outpatient team-centered team, and family and/or natural supports, in order to develop and implement the discharge plan.
  • Completes all necessary discharge planning documents and submits documentation and referral packages in a timely manner to the RBHA and community providers.
  • Coordinates with the Copa care team to ensure scheduling of post-discharge appointments within 72 hours of the discharge. Ensures follow-through with discharge recommendations and services, and identifies gaps in services
