Hospital Clinical Liaison
About The Position
PurposeCare is seeking a relationship-driven and operationally strong Care Transition Nurse / Hospital Liaison to support hospital partnerships and help patients transition smoothly into home health services. This role is ideal for someone who thrives in a fast-paced healthcare environment, enjoys building strong relationships with hospital teams, and understands the importance of urgency, communication, and patient advocacy during care transitions. As a key member of our team, you’ll work directly with hospital case managers, discharge planners, physicians, patients, and families to coordinate referrals, support admissions, and improve continuity of care.
Requirements
- LPN or RN required
- Minimum of 3 years of experience in: Home health, Care transitions, Clinical liaison work, Hospital case management, Healthcare operations
- Strong relationship-building and customer service skills
- Ability to manage multiple priorities in a high-volume healthcare environment
- Strong attention to detail and follow-through
- Strong critical thinking and communication skills
- Comfortable working independently in the field while collaborating closely with operational teams
Nice To Haves
- Experience working with referral platforms and EMR systems preferred
Responsibilities
- Coordinate home health referrals and patient transitions from hospitals into home-based care
- Partner closely with hospital case managers, discharge planners, social workers, and healthcare teams
- Manage and follow up on referrals with urgency and strong attention to detail
- Educate patients and families on home health services and available support options
- Conduct regular onsite hospital visits and maintain strong referral relationships
- Collaborate with PurposeCare Intake and Clinical Operations teams to support timely Starts of Care
- Utilize systems such as: CarePort, WellSky, Axxess, Trella
- Help identify opportunities to improve referral conversion and patient experience
- Support awareness of both home health and private pay service offerings
