Hospital-Based Hospice Nurse Liaison
About The Position
Named by the Boston Globe as one of the Top Places to Work in Massachusetts for the last four years, our community based, mission-driven hospice and palliative care organization seeks a Hospital-Based Hospice Nurse Liaison to join its expanding Intake and Admissions team. This is an on-site position located at Newton-Wellesley Hospital. The position of Hospital-Based Hospice Nurse Liaison (HBHNL) is to provide information about hospice services to patients, families, and hospital-based staff, to conduct clinical assessments, coordinate hospital discharge plans, and maintain professional relationships within hospitals, with a focus on the assigned hospital.
Requirements
- Licensed by the Commonwealth of MA as a Registered Nurse
- At least two years experience preferred in community health, oncology, hospice, or palliative care
- Excellent communication and interpersonal skills
- Ability to work collaboratively with hospital staff, Good Shepherd colleagues, patients, and their families
- Highly skilled at providing patient assessments and evaluations
- Excellent problem-solving skills
- Competency with accepted standards of hospice nursing practice
