About The Position

The Hospital at Home (H@H) Acute Care/MACU (mobile acute care unit) RN Team Leader is responsible for assisting with reviewing patients in EMR with H@H case manager and social worker for program eligibility. The MACU RN Team Leader provides coordination of homecare services, care, and documentation of care for program patients in accordance with Quality Standards and under the guidance of a clinical supervisor. This role ensures the quality and safe delivery of acute hospital care at home services. Responsibilities include screening hospital patients for program eligibility, scheduling patients to MACU nurses and therapists, and educating patients and families about the program and expectations. The Team Leader facilitates communication among departments, agencies, and an interdisciplinary team of clinicians, and coordinates internal and external resources to meet patient needs. Clinical care is provided as needed to assigned patients in the private home environment, which requires a minimum of 2 nursing visits per day, use of telehealth monitoring and virtual visits, and joint visits with medical providers as needed. Collaboration with medical providers at hospitals is essential, including communicating any changes in patient condition, routine clinical updates, and progress toward goals. The role involves administering medications and care to patients as ordered and maintaining skill competency for complex care delivery, including proficient assessment skills, high-tech IV skills, procedural skills, and equipment use (e.g., catheter care, drainage tubes, oxygen, durable medical equipment, Telemonitoring, etc.).

Requirements

  • Current license as a Registered Nurse in New York State.
  • Ability to provide care to individuals in private homes.
  • Strong written and verbal skills with ability to manage stressful situations including conflict resolution.
  • Excellent computer knowledge with proficiency in Microsoft Office, or other word processing, spreadsheet, and database management
  • Excellent organizational skills and ability to prioritize multiple tasks.
  • Efficient and flexible in problem solving with ability to motivate and educate others.

Nice To Haves

  • Bachelor's degree is preferred from an approved school, acceptable to the New York State Department of Health, preferred.
  • Experience as a clinician with home and community-based services preferred.
