Requirements

  • Must be a graduate from an accredited School of Nursing
  • Must be licensed in the State as a Registered Nurse
  • Must have knowledge of Medicare and Medicaid guidelines
  • Must have a criminal background check
  • Must have a current CPR (BLS) certification
  • Ability to work independently and make appropriate clinical decisions within the scope of practice.
  • Strong organizational skills and attention to detail.
  • Proficiency with electronic medical records and standard computer systems.
  • Compassionate, professional, and patient-centered approach to end-of-life care.

Nice To Haves

  • One or more years of experience in hospice preferred

Responsibilities

  • Answer incoming calls from hospice answering service, families, caregivers, facilities, physicians, and field staff.
  • Perform comprehensive telephone triage and assess changes in patient condition, pain, medication concerns, and psychosocial needs.
  • Determine the appropriate level and urgency of care based on the patient's condition and hospice plan of care.
  • Provide education, reassurance, and clinical guidance to patients and caregivers within the scope of hospice practice.
  • Escalate urgent or complex clinical concerns to the hospice medical director, provider, or appropriate clinical administrator on call.
  • Recognize situations requiring an in-person visit, increased level of care, emergency intervention, or other appropriate action.
  • Document all triage calls, assessments, interventions, and follow-up recommendations accurately and timely.
  • Serve as a clinical resource and support for nurses and other field clinicians throughout the shift.
  • Direct field nurses to patient visits based on acuity, geographic considerations, staffing, and patient needs.
  • Coordinate PRN, urgent, and symptom-management visits.
  • Communicate changes in patient status and care priorities to the appropriate members of the interdisciplinary team.
  • Assist field staff with clinical questions, symptom management, medication concerns, and care-plan interventions.
  • Receive and coordinate new hospice referrals from hospitals, physicians, facilities, agencies, and other referral sources.
  • Obtain and review clinical information to determine appropriateness for hospice evaluation in collaboration with the appropriate clinical leadership and/or medical director.
  • Coordinate hospice evaluations and admissions with patients, families, facilities, referral sources, and field clinicians.
  • Communicate referral information and patient needs to the appropriate admissions/evaluation nurse.
  • Ensure necessary clinical documentation and information are obtained for timely processing of referrals.
  • Track referrals and follow up on pending admissions to support timely conversion and continuity of care.
  • All other duties as assigned.
