About The Position

Patriot Hospice is seeking a Registered Nurse Case Manager for a weekend hospice role serving Trumbull, Mahoning, and Columbiana Counties. The schedule is Friday through Sunday, 8:00 a.m. to 8:00 p.m. The role coordinates hospice care with patients, families, and other team members, and includes start-of-care evaluations, Service Intensity Add-On visits, triage, and on-call duties. Compensation is hourly, with additional earning potential from picking up visits at the per diem rate.

Requirements

• Graduate from an accredited School of Nursing.
• Licensed as a Registered Nurse in the state.
• Knowledge of Medicare and Medicaid guidelines.
• Criminal background check.
• Current CPR certification.
• Valid driver's license and willingness and ability to drive to patients' residences.
• Ability to access patients' homes, which may not be routinely wheelchair accessible.
• Hearing, eyesight, and physical dexterity sufficient to perform physical assessments and perform and demonstrate patient care.
• Ability to walk, sit, stoop, and stand, and to lift and turn patients, from minimal to maximum lifting.

Nice To Haves

• One or more years of hospice experience.

Responsibilities

• Coordinate hospice care with team members, patients, and family members.
• Perform start-of-care evaluations and Service Intensity Add-On (SIA) visits.
• Triage patient calls and complete visits as needed.
• Evaluate the effectiveness of hospice services for patients and families on an ongoing basis.
• Perform hospice care evaluations and informational visits as needed.
• Submit complete and accurate clinical notes in a timely manner.
• Participate in IDG and case conferences.
• Perform on-call duties, including holidays, per policy and agency need.
• Perform other duties as assigned.
