About The Position

Grace Hospice is seeking a compassionate, dependable Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join their hospice team and provide skilled nursing support to patients and families during weekend and after-hours shifts. As a nonprofit hospice organization, Grace Hospice focuses on the patient and family, emphasizing comfort, dignity, quality of life, and compassionate end-of-life care. The Hospice RN/LPN works collaboratively with the triage nurse and interdisciplinary hospice team to provide skilled nursing visits to patients and families requiring after-hours support. This position responds to patient needs in the home or care setting, identifies and reports changes in condition, assists with symptom management, provides patient and caregiver education, and communicates with the hospice care team to ensure timely and compassionate care. Responsibilities are performed within the nurse's applicable scope of practice, licensure, and Grace Hospice policies and procedures. Saturday and Sunday shifts are available.

Requirements

  • Graduate of an accredited school of nursing
  • Current and unrestricted Minnesota RN or LPN license
  • Minimum of three years of recent nursing experience in acute care, long-term care, home care, hospice, or public health preferred
  • Strong patient care, observation, care planning, and patient/family education skills appropriate to scope of practice
  • Excellent written and verbal communication skills
  • Strong critical-thinking and clinical decision-making abilities
  • Ability to work independently while collaborating effectively with the interdisciplinary team
  • Ability to recognize changes in patient condition and communicate concerns promptly
  • Current Minnesota driver's license and automobile insurance
  • Daily access to a dependable vehicle for patient visits
  • Basic computer proficiency and ability to complete electronic clinical documentation
  • Compassionate, dependable, and comfortable supporting patients and families during end-of-life care

Responsibilities

  • Provide compassionate, respectful, and professional care to patients and families.
  • Collaborate closely with the triage nurse and complete face-to-face patient visits as directed based on patient and clinical needs.
  • Observe and assess patients within the nurse's scope of practice for changes in condition, symptom-management needs, and other clinical concerns.
  • Provide skilled nursing interventions within the nurse's scope of practice to promote patient comfort and quality of life.
  • Promptly communicate significant findings, changes in condition, patient or family concerns, and changes in the care environment to the appropriate physician, Clinical Manager, triage nurse, and other members of the hospice interdisciplinary team.
  • Obtain, receive, communicate, and implement physician orders as appropriate and within scope of practice and agency policy.
  • Educate patients, families, and caregivers regarding medications, symptom management, comfort measures, safety, and the hospice plan of care within scope of practice.
  • Administer medications and treatments as ordered and permitted by licensure and agency policy.
  • Complete clinical documentation accurately and within established timeframes.
  • Communicate promptly with leadership when patient needs, visit expectations, or assigned responsibilities cannot be completed as expected.
  • Work collaboratively with other hospice team members to promote continuity and quality of care.
  • Maintain patient confidentiality and comply with HIPAA requirements.
  • Demonstrate knowledge and understanding of patient rights, vulnerable adult requirements, and hospice standards of care.
  • Demonstrate sound judgment and appropriately escalate urgent or changing patient situations.
  • Seek and accept feedback regarding job performance and opportunities for professional growth.
  • Consistently provide high-quality, patient-centered end-of-life care.
