About The Position

This is a part-time Registered Nurse position focused on providing hospice care on weekends. The schedule is Saturday and Sunday from 8 am to 6 pm, with full-time availability if desired. Requirements

Graduate of an accredited nursing program
Current licensed Registered Nurse (RN) in the state of practice
Valid driver’s license and reliable transportation
Proof of automobile insurance (MVR checked)
Proof of current CPR certification
Able to pass criminal background check and national sex offender clearance

Nice To Haves

One year of experience in an acute-care setting within the last 24 months
One year Home Health care experience within the last 24 months
One year community health or public health experience within the last 24 months
One year Hospice experience within the last 24 months

Responsibilities

Provides services requiring substantial and specialized nursing skill, in accordance with the plan of care.
Manages caseloads varying from approximately 12-18 patients, depending on complexity, geography, and setting.
Demonstrates competency in skills such as pain management, symptom control, infusion therapy, infusion therapy device care, wound care, and aseptic technique.
Applies critical thinking, analysis, and clinical judgment in caring for patients and families facing terminal illness.
Addresses the physical, emotional, psychosocial, and spiritual needs of the patient and family/caregiver.
Makes referrals to other disciplines as needed.
Coordinates the total Plan of Care and maintains continuity of patient care by liaising with other health professionals.
Attends all interdisciplinary group meetings.
Initiates patient care conferences for complex patients.
Includes the patient/family in the implementation of the patient care plan.
Evaluates and routinely re-evaluates the nursing needs of the patient, and makes necessary revisions to the Plan of Care.
Educates the patient and family/caregiver on their rights and responsibilities as a Hospice patient.
Communicates with the patient's Attending Physician and the Hospice Medical Director regarding changes in condition.
Acts as a Case Manager for the patient's care, ensuring equitable and beneficial utilization of resources.
Manages patient care from admission through discharge and coordinates with the interdisciplinary team on bereavement.
