The Hospice Weekend Coordinator is responsible for responding to all requests in a positive manner, understanding the skills and services provided by the multidisciplinary team, and relating them to identified patient needs. This role involves prioritizing and assigning referrals based on patient needs, understanding and integrating hospice standards and regulations, and accepting and documenting verbal orders. The coordinator inputs initial information into Epic to begin the admission process, elicits and gathers accurate patient information, and forwards referral information to the appropriate team. They respond to telephone requests, interpret services, triage patient calls, advise interventions, and act as a resource to patients, families, and caregivers. Collaboration with the weekend Administrator On Call (AOC) is essential for addressing patient-related issues, triaging unanticipated needs, assigning visits, and scheduling patient visits. Communication with physicians, timely communication to the clinical team, and adherence to confidentiality principles are critical. Documentation requires attention to detail, proficiency with office software including Epic, and adherence to agency timeliness policies. Professional development includes maintaining and updating clinical knowledge, adhering to departmental training requirements, and identifying learning needs. Additional duties involve participating in weekend/holiday rotations, in-service programs, and supervising/teaching hospice nursing staff.
